Locks Heath Hypnotherapy Privacy Notice
I, Caroline DOYLE, take your privacy seriously, and in accordance with the General Data Protection Regulation, I will commit to the following:
I will be asking you for personal data about you in order to deliver a professional hypnotherapy and emotional freedom technique service to you. Due to the nature of the service provided some of this information is sensitive personal data and is confidential. I must have a legal basis for collecting this data, and there are six lawful bases:
(a) Consent:
The individual has given clear consent for you to process their personal data for a specific purpose.
(b) Contract:
The processing is necessary for a contract you have with the individual, or because they have asked you to take specific steps before entering into a contract.
(c) Legal obligation:
The processing is necessary for you to comply with the law (not including contractual obligations).
(d) Vital interests:
The processing is necessary to protect someone’s life.
(e) Public task:
The processing is necessary for you to perform a task in the public interest or for your official functions, and the task or function has a clear basis in law.
(f) Legitimate interests:
The processing is necessary for your legitimate interests or the legitimate interests of a third party unless there is a good reason to protect the individual’s personal data which overrides those legitimate interests. (This cannot apply if you are a public authority processing data to perform your official tasks.)
I will be processing your data under the following bases: (a) Consent, (b) Contract, (c) Legal obligation
Where I require consent, I will provide a way for you to positively make a decision about the information that you make available and how this is shared. This is on the terms and conditions form that is part of the contract made between us.
This information will be collected by myself as part of your initial enquiry on the phone, facebook messaging/post, www.locksheathhypnotherapy.co.uk, email or face to face.
It will also be collected at your initial hypnotherapy session. I will be asking for the data that is underlined at the initial point of contact. I will be asking for all of the points below at our initial session recording it on paper forms which will form your case notes. These notes are then stored in a locked filing cabinet with the keys accessible only to me.
The information that I require will be:
- Your name
- Your date of birth
- Your address
- Your phone number
- Your email address
- Your doctor’s name and contact number
- Your occupation
- Any allergies/medical history/ requirements/ health concerned
- Names of your partner/children/pets
- Your fears/phobias
- Your favourite place or a place special to you
- Your goals and ambitions
- Other information relevant to your hypnotherapy treatment
I am required to practice safely and ethically by the National Hypnotherapy Society Code of Conduct. This data is used to enable the safe and ethical practice when hypnotherapy or emotional freedom technique is used to ensure the therapist has relevant information about the client. This data will be used to:
- assist you to receive the most appropriate therapy
- support you through your chosen therapy
- work collaboratively with you to assist you to achieve your chosen goals
- share information about activities in our setting
- share with other professionals in accordance with legislation
- ensure a contract of service is delivered and maintained
In the case of an emergency, your information may be shared with emergency services personnel.

In the event of you needing to update or amend information held by Locks Heath Hypnotherapy email lhhypnotherapy@outlook.com
Your data is stored securely in a locked filing cabinet or on password-protected devices and will be retained for a period of 5 years after the end of your last session. All paper records will then be securely destroyed.
